Referral Process
for
All Private Insurances, Self-Payment, &
Medicaid Standard PHP Plans
There are 3 ways to refer to our agency:
1. Call our agency and self-refer. We will obtain your information and provide you with an appointment.
2. Print & complete the referral form (below); then, fax it to our office at 910-299-0850. Once this referral form is received and reviewed, you will be contacted to schedule your appointment.
3. Physician's office must complete the referral form below or fax your agency referral form along with supporting documentation to 910-299-0850.
